Through our case management and delegation services, we will evaluate our client’s condition and circumstances then, determine what care options, supports, and other programs will be suitable for the client and discuss this with their family. Also, they serve as a bridge to establish proper communication between the client’s family and the care team.

Whether it is recommended that the client stays at home or in an independent living facility, our case managers will work closely with the client, their families, and the care team to ensure that the needs and preferences of the client are met.
